As an Early Childhood Early Intervention Coordinator, you will be part of a multidisciplinary team which supports families with children 0 to 6 years old (inclusive) who are exploring their eligibility for NDIS funding.
Reporting to the Early Childhood Early Intervention Regional Lead the ECEI Coordinators will be responsible for screening and assessing children with a developmental delay and/or disability to determine their eligibility for NDIS funding. The ECEI Coordinators will develop and review plans for participants in the scheme and provide short term interventions to build the capacity of families to support their child’s development where they have been deemed ineligible for NDIS funding. You will also be responsible for the preparation, submission and (where agreed) facilitation of the implementation and ongoing review of NDIS plans.
Your key responsibilities will include;
- Conduct client referral appointments, administer screening and functional assessments to determine support requirements, funding eligibility and referral pathways.
- Develop families understanding and knowledge of the NDIS planning process to support their ability to make informed decisions.
- Support families to understand the eligibility requirements, functional goals and reasonable/necessary supports.
- Recognise and respect the diversity of families and styles of communications.
- Identify and document the child and family’s functional goals.
- Develop and submit client plans and supporting information to the NDIA to determine funding entitlements.
- Work with families to determine approach to the NDIS plan implementation and management.
- Participate in networking and maintain effective linkages with relevant services and agencies in the community.
- Build and maintain effective relationships with external service providers.
- Conduct regular assessments to monitor and evaluate client progress towards plans.
- Work with the team to develop appropriate short to medium term family centred interventions to address children’s and family’s functional goals.
The ideal candidate will be degree qualified in social work, early childhood education or allied health, have solid computer skills, possess strong written and verbal communication skills, have an understanding of a family centred approach, be a team player, be flexible, action orientated and have an innovative approach to their work.
Experience working within the early childhood or disability sector is essential and eligibility for membership of appropriate professional bodies – e.g. AHPRA, Speech Pathology Australia is preferred. A valid driver’s licence and capacity to undertake outreach work is required.
